Coventry Health Care of Kansas, Inc., a leading managed health insurance company, has an immediate opening for a professional RN to join our team. This position will be responsible for implementing and coordinating case management activities relating to catastrophic cases and chronically ill members. Utilization of case management concepts and strategies to improve member knowledge and self management of their condition. May be responsible for developing and executing implementation plans with a physician practice to coordinate all case management activities across the continuum of care including consultant referrals, home care visits and use of community resources. Uses computer-based systems to review medical experience of members and interact with plan staff.

Position Requirements:
-Registered nurse with current MO and KS state license;
-Bachelor Degree or equivalent experience preferred;
-Complies with all state certification requirements in the state where job duties are performed;
-Previous experience (usually 1+ year) in case management and/or discharge planning;
-Significant (usually 3+ years) clinical experience;
-Utilization management experience and knowledge of community resources preferred;
-Experience using computer systems as part of performing case management activities;
-CCM preferred
